Tax codes are issued by HMRC directly. There are a number of reasons why HMRC may change your tax code.

HMRC's goal is to ensure that everyone has paid the correct amount of income tax by the time we reach year end every April so that means they may need to make adjustments to your code.
If you are employed by the same, single employer each year then it is less likely your tax code will change (as they can track your earnings very easily). There are however some recurring reasons we see for tax code changes:
HMRC may update your tax code if:

you start to get income from a new job, an additional job or even a pension

your employer tells HMRC you have started or stopped getting benefits from your job

you get taxable state benefits

you claim Marriage Allowance

you claim expenses that you get tax relief on
- HMRC predict your earnings to exceed the £100k mark, which reduced your tax free allowance by £1 for every £2 earned over 100k

you may also be put on an emergency tax code if you change jobs or in that transition period between two jobs
Whilst HMRC make every effort to track everyone's movements and earnings, if a mistake has been made they're usually good at fixing this (usually it just takes a quick conversation to explain your current situation and sources of income)
